Hello everyone! Today, we're going to look at how you can implement forms into your workflow with Edify. This video will demonstrate how to create an uploadable Form and a Learner Declaration. Let’s begin with looking at Uploadable forms.
1. Creating an Uploadable Form
Whether you need learners or trainers to submit completed checklists, forms, or signed agreements, creating an uploadable form is quick and simple.
Step 1: Add an Attachment Section
- Navigate to your unit’s Online Variant.
- Select either the Assessments tab or the Supporting Info tab.
- Click the + (Add) button to create a new item.
- In the pop-up window, click the Section type dropdown menu and select Attachment.
- Enter a Title for your attachment.
- Click the paperclip icon to open the Media Library, select your desired document (e.g., PDF), and click Insert.
- Click Save to create the section.
Step 2: Enable the Uploadable Form Setting
- Locate your newly created attachment section (it will appear as the last section in the tab).
- Click the cog icon (edit settings) on the bottom right of the section.
- In the Section Editor window, check the box next to Uploadable Form. The window will expand to reveal additional content and permission settings.
- In the Content text field, type your instructions for the user.
- Include the required tag {{ATTACHMENT}} in your text where you want the download link to appear. Note: it will appear in your text as a link behind the word “form”.
Important: You must include the exact tag {{ATTACHMENT}} within your text. Edify relies on this specific tag to automatically generate the document's download link and the upload box for the user.
Example Instruction Text:
"Please download the {{ATTACHMENT}}, fill it out, and re-upload the completed PDF." Under the "Who can upload form?" section, check the boxes for who has permission to upload completed files:
- Learner
- Trainer
(You can select either or both depending on your workflow requirements.)
- Click Save.
- Click into the published section to preview it.
- The download link will display as the word "form" in blue text.
- The file upload box will display to the right.
Tip: Always review your form before marking your unit as Ready to ensure everything functions properly.
2. Adding a Learner Declaration Form
Learner Declaration forms are a great way to confirm that learners have read, understood, and agreed to your academic honesty guidelines before submitting their work.
To create a new Learner Declaration Form:
- Go to the Online Variant of your unit.
- Select either the Assessments tab or Supporting Info tab.
- Click the + (Add) button.
- From the dropdown menu, select New Declaration Form.
- Enter a clear Title (e.g., Learner Declaration).
- Enter any Introductory Text for the learner (e.g., "Check the box to indicate that you agree").
- Click + Declaration items to begin adding checklist requirements.
- Type out each statement the learner must agree to.
Example Declaration Checklist Items:- I have read all instructions and understand what I need to submit
- All work submitted is my own
Tip: Only add items that are mandatory for the learner to accept. Learners are required to check every single checkbox on the form before they are allowed to submit their work
- (Optional) If you plan to use this exact declaration in other units, check the box for "Save Declaration Form to be used again".
- Click Save.
- Click on the completed section to review the checkboxes and ensure the form displays correctly.
