  What is a Declaration Form?

A Declaration Form is a checklist of items that the Learner must agree to by checking the boxes. Declaration forms are used to confirm students have read, understood, and will follow the academic honesty guidelines.

  Note: All items must be agreed to for the Section to be marked as complete by the system.

How to Build a Declaration Form

  1. Click the + symbol and select New Declaration form from the drop-down menu. (You can select Saved Declaration forms to access forms you have previously created and saved.)

  2. The pop-up window will expand. You will need to add a Title and the Declaration Intro text in the text box.

  3. Click the + Declaration items button to add items to your declaration.

    Once you have added your items, click Save to save your Declaration. You can check the box next to Save Declaration Form to be Used Again to save the form to use in future custom assessments.
